The pediatric dosage can be calculated as follows.  Example:  The physician orders a drug that has a recommended dosage of 30mg/kg/day.  How much would you give a 90-lb child each day?

1.  Convert 90-lb to kilograms.

            90-lb  x   0.45kg /1-lb  = 40.5 kg

2.  Multiply weight in kilograms by the ordered dose.

            40.5 kg  x  30 mg/day / 1kg  = 1215 mg/day

3.  If the orders are written for b.i.d., the dose would be divided by 2.

            1215 mg/day /  2 = 608 mg/dose
